In addition to the registration forms, three pieces of documentation are required for all new students:

1. Proof of Residency (lease with address, mortgage with address, utility bill, driver's license or auto registration).

2. Student's Birth Certificate (per Public Law 962)

3. Proof of Custody (court documentation must be provided if student is not residing with both natural parents).
